 ments in the atrium\nAbstract\nThis talk spans some of our activity from r
 are to prevalent diseases\, bringing together collaborative intelligence a
 nd longitudinal prediction. We will show how the Share4Rare data science b
 ackend structures patient-reported data and uses graph-based “people-lik
 e-me” similarity to route questions and surface expertise in the context
  of deep phenotyping. We then will move  to our work with routine health r
 ecords\, where deep sequence models learn representations to forecast risk
 s\, treatment changes\, and HbA1c targets in type 2 diabetes\, revealing m
 eaningful patient trajectories\, paving the path to our first Foundation M
 odels in Health.\nBio\nAlexandre Perera i Lluna is a physicist and electri
 cal engineer with a PhD in Physics. He is a Full Professor at the Universi
 tat Politècnica de Catalunya (UPC) and Coordinator of the b2slab – Bioi
 nformatics and Biomedical Signals Laboratory (b2slab.upc.edu). His researc
 h focuses on applied data science methods for clinical data and bioinforma
 tics. He currently serves as Director of the Catalan health-tech network X
 arTEC Salut (xartecsalut.com). He has also co-founded three spin-off compa
 nies.
